Most people who come to us for decompression are not in their first week of back pain. There is usually a bulge or a herniation already on a scan, sometimes years old. Rest, medication and occasionally an injection have all been tried, and the conversation has started to turn towards surgery. That is exactly the point at which a structured course of conservative care is worth running properly, because for lumbar disc herniation with radiculopathy the conservative options carry moderate evidence, and the ACC pathway asks for them before invasive options.

Decompression applies slow, controlled axial distraction to the segment. Separating the vertebral bodies lowers the pressure inside the disc and widens the opening the nerve root travels through, and that is the part that takes mechanical load off an irritated root. The further idea that the drop in pressure also draws fluid and nutrients back into the disc is plausible but still debated in the literature, so we do not build a plan around it.

If you already have imaging, bring the report or the images. A known level changes how the distraction is set up and what we watch for between visits. If it began with an injury, bring the ACC claim number or we will lodge it for you. And the honest limit: progressive weakness, numbness through the saddle area, or any loss of bladder or bowel control is not a case for conservative care, and we refer the same day.
