5 Signs You Need a Chiropractor (Not a Physio)
Not all musculoskeletal pain needs the same treatment. Chiropractors and physiotherapists are both highly trained practitioners — but they approach pain very differently. Understanding which one to see first could mean the difference between months of frustration and a clear path to recovery.

Chiropractor vs Physio — Why the Distinction Matters
In my 12 years of practice, one of the most common questions I hear from new patients is: “Should I have gone to a physio instead?” Sometimes the answer is yes. But more often, patients who have spent months at physiotherapy come to us because their pain keeps returning. There is a structural issue at the root that exercise rehabilitation alone cannot resolve.
Here is the core difference: physiotherapists primarily work on restoring function and strength through exercise and manual therapy. Chiropractors focus on identifying and correcting structural and joint misalignments — particularly in the spine — that create nerve interference, biomechanical imbalance, and recurring pain patterns.
Neither is superior. They address different root causes. The right choice depends on what is actually driving your pain. Below are five signs that chiropractic is likely the more appropriate first step.
The 5 Signs
You had a flare-up, rested, felt better, returned to normal life — and then it happened again. Maybe it is the same lower back episode every few months, or a neck that seizes up under stress. If this pattern sounds familiar, you are likely dealing with a structural problem, not just a muscle strain.
Muscle strains heal. Spinal joint restrictions, disc issues, and vertebral misalignments do not resolve on their own — they create ongoing abnormal load on surrounding muscles and nerves, which is why the same area keeps flaring up. Each episode may even be slightly worse than the last as compensation patterns deepen.
A chiropractor will assess the full spinal structure — not just the painful area — to identify the underlying cause of the recurring cycle. Correcting that structural cause is how you break the pattern, rather than managing each episode as it arrives.
Numbness, tingling, pins and needles, or weakness travelling down an arm or leg are not just muscle symptoms — they indicate nerve involvement. This typically means a spinal structure (a misaligned vertebra, a bulging disc, or a narrowed foramen) is placing pressure on a nerve root.
The sciatic nerve — the longest nerve in the body — runs from the lumbar spine through the buttock and down the leg. Compression at the L4, L5, or S1 level produces the classic sciatica pattern: radiating pain and numbness down one leg, sometimes as far as the foot. Similarly, cervical nerve compression produces symptoms into the shoulder, arm, or hand.
Chiropractic is specifically trained to locate and reduce nerve compression through spinal adjustment and decompression techniques. Where disc involvement is significant, spinal decompression therapy — offered at Auckland Wellness Centre — can provide substantial relief by creating negative intradiscal pressure that draws the disc material away from the nerve.
If numbness or tingling is accompanied by bladder or bowel changes, seek emergency care immediately. This is a rare but serious neurological emergency.
You may have noticed it in photos, in shop window reflections, or because someone else pointed it out — your shoulders are rounding forward, your head is drifting in front of your body, or one shoulder sits higher than the other. Postural deterioration is not a cosmetic issue; it is a structural one with genuine health consequences.
For every inch the head moves forward of its neutral position, the effective weight on the cervical spine approximately doubles. A head that normally weighs 4–5 kg can exert the equivalent of 18–27 kg of force on the neck when significantly forward. This accelerates disc degeneration, causes chronic muscle fatigue, and creates a feedback loop of worsening posture and pain.
Correcting posture requires addressing the spine, not just strengthening surface muscles. Chiropractic care — particularly Chiropractic BioPhysics (CBP), practised by Dr. Michael Blandy at AWC — focuses on measurably correcting spinal curvature and alignment using specific traction protocols, mirror image adjustments, and corrective exercise.
Physiotherapy can improve muscle strength and body awareness, which is complementary to structural correction. But the structural component requires chiropractic assessment first.
The average New Zealand office worker spends 7–10 hours a day seated, often in a position that loads the spine unevenly. Over months and years, this creates predictable damage patterns: anterior head carriage (text neck), lumbar disc compression, and thoracic kyphosis (rounding of the upper back).
Text neck — formally called cervical kyphosis — is the reversal of the normal cervical curve caused by prolonged forward head posture during device use. It is responsible for a significant portion of the chronic neck and headache presentations we see at Auckland Wellness Centre, particularly in patients aged 25–45.

The intradiscal pressure when seated is measurably higher than when standing, and the compressive load is worsened by poor lumbar support and forward lean.
If you are experiencing tension headaches, neck stiffness, or aching lower back after your workday, these are early warning signs of structural loading that chiropractic assessment and correction can address before they progress to disc pathology or chronic pain.
Monitor at eye level, elbows at 90 degrees, lower back supported, screen 50–70 cm away. Stand up for at least 5 minutes every 45 minutes. A chiropractic assessment can identify which spinal levels are already affected.
Whether it was a car accident, a sports collision, a workplace incident, or a fall, any sudden impact event can cause spinal joint subluxations — misalignments that do not always produce immediate pain. This is one of the most misunderstood aspects of accident-related injury: the absence of immediate pain does not mean no structural damage occurred.
Whiplash is the most well-documented example. In a rear-end collision, the head and neck undergo rapid deceleration forces that can strain ligaments, misalign cervical joints, and compress disc structures — all before the muscles have had time to react. Symptoms often appear 24–72 hours after the event, or even weeks later, by which point acute inflammation has set in and early management opportunities have been missed.
Seeing a chiropractor promptly after an accident achieves two things. First, it provides a thorough spinal assessment to identify any structural damage that imaging may not immediately reveal. Second, early mobilisation and adjustment of affected joints has been shown to significantly reduce the risk of chronic pain developing from acute injuries.
In New Zealand, you can access ACC-funded chiropractic care directly without a GP referral. The ACC surcharge at Auckland Wellness Centre is $40 per visit for chiropractic. Our team will handle the ACC45 claim form at your first appointment.
When a Physio Is the Better Choice
Intellectual honesty matters in healthcare. There are situations where physiotherapy is genuinely the more appropriate first choice, or where physiotherapy and chiropractic should work in parallel. Here is a balanced view:
- Recurring pain with no lasting resolution
- Numbness, tingling, or weakness into limbs
- Postural deterioration and structural issues
- Neck pain with associated headaches
- Disc bulge or herniation diagnosis
- Accident-related spinal injury
- Sciatica or nerve root compression
- Post-surgical rehabilitation
- Specific muscle weakness or atrophy
- Neurological rehabilitation after stroke
- Balance and coordination disorders
- Respiratory physiotherapy needs
- Pelvic floor dysfunction
- Hydrotherapy requirements
Many complex presentations benefit from an integrated approach. A chiropractor corrects the structural issue; a physiotherapist then builds strength and movement patterns around that corrected structure. At Auckland Wellness Centre, our team will advise honestly if a referral to physiotherapy would complement your chiropractic care plan.
What Happens at Your First Chiropractic Visit
Many patients delay chiropractic care because they are unsure what to expect. At Auckland Wellness Centre, your initial appointment includes a comprehensive health history, postural and orthopaedic assessment, neurological screening, and a full explanation of your findings before any treatment begins. We will never treat without your informed consent and a clear understanding of your care plan.

Frequently Asked Questions
Chiropractors focus on diagnosing and correcting spinal and joint misalignments (subluxations) that affect the nervous system and structural health. Physiotherapists focus on restoring movement, strength, and function through exercise rehabilitation and manual therapy.
Chiropractic is often better suited to recurring structural pain, nerve symptoms, and postural correction. Physio is often better for post-surgery rehab and targeted muscle rehabilitation.
Yes. Neck pain is one of the most common conditions treated by chiropractors, especially when it involves stiffness, recurring headaches, or numbness and tingling into the arms. Chiropractic adjustments can restore cervical joint mobility, relieve nerve pressure, and correct postural patterns that perpetuate the pain.
Yes. Chiropractic is covered by ACC for accident-related injuries. You can see an ACC-registered chiropractor directly without a GP referral. The ACC surcharge at Auckland Wellness Centre is $40 per visit for chiropractic care.
Many patients notice meaningful improvement within 3–6 visits. Acute injuries often respond quickly. Long-standing structural issues take longer to correct and typically require a structured care plan. Your chiropractor will give you a realistic expectation at your initial assessment.
If your lower back pain is recurring, accompanied by leg numbness or tingling, or related to a disc issue, a chiropractor is often the more appropriate first choice. If you need post-operative rehabilitation or specific muscle strengthening, physio may be more suitable. Many patients benefit from both at different stages of care.
Yes. Chiropractic is one of the safest manual therapies available. Registered chiropractors in New Zealand hold a 5-year university degree and are regulated by the New Zealand Chiropractic Board. Serious adverse events are extremely rare. Your chiropractor will conduct a thorough assessment and take a full health history before any treatment.
